Abstract
결론
저자 등은 1994년 11월부터 1995년 9월까지 하악 과두골절로 진단 받은 12명의 환자에 대
하여 Lag Screw를 이용하여 관혈적 정복 수술을 시행한 후, 평균 약 10개월의 주기적인 검
진 결과 다음과 같은 결과를 얻었다.
1. 술 후 개구량은 1명을 제외한 나머지에서 모두 40mm이상의 결과를 얻었다.
2. 1명을 제외한 악관절의 기능은 모두 만족할 만 하였으며, 약간의 관절 잡음이나 개구시
하악의 변위 등은 관찰할 수 있었으나, 개구시 동통이나 기타 악관절 장애는 나타나지 않았
다.
3. Screw의 하방 이동과 골절편의 흡수는 각각 2명의 환자에게서 보였으나, 만족할 만한 기
능 회복을 유도하였다.
이와 같은 결과로 lag screw osteosynthesis는 선택된 하악 과두 골절 중 Level Ⅲ 또는
Ⅳ에서 비교적 만족할만한 결과를 보였다.
#초록#
Treatment of condylar fracture is still controversial, but the indication of surgical
management is increased due to improved fixation methods and surgical techniques. In
this article, we report our clinical and radiological results with the use of lag screw
osteosynthesis for fixation of fractured mandibular condyle. 12 cases of condyle fracture
treated by open reduction and fixation with lag screw were monitored for an average of
10 months (range from 6 months) postoperatively. The obtained results are as follows :
1. Postoperative maximum mouth opening is achieved over 40mm except one patient.
2. Although slight crepitus and deviation of mandible during mouth opening were found.
Temporomandibular joint problem is satisfactory in all but one patient.
3. Distal migration of screw and resorption of fractured fragment were found in each 2
patients, but good functional recovery was achieved in all.
According to the above results, we think that lag screw osteosythesis is relatively
indicated in level Ⅲor Ⅳ condylar fracture
